Click below to download our "New Patient Paperwork". Please have your paperwork completed prior to your appointment.  It will help to expedite your clinic visit.

If you are having difficulty downloading the new patient paperwork, please call the office so the paperwork can be faxed or mailed to you.

Please remember:

1) To arrive 15 minutes prior to your appointment time with your new patient paperwork.

2) To bring you driver's license and insurance card with you.

3) For HMO and PPO patients, please call your insurance company prior to your visit to verify that your physician is a covering provider. If you have a HMO or Tricare, please verify that your physician is listed as your primary physician.

*If you are unable to make it to your appointment, please call the office as soon as possible (at least 24 hours in advance) to cancel your appointment. New patient appointments are given longer time slots and your cancellation will free the appointment time for sick patients to be seen.   A fee may be charged for late cancellation.
