PRESCRIPTION REFILLS
Please call your pharmacy to have refill requests faxed to the clinic. Please allow up to 48 hours for refills to be approved. Refills are done during normal business hours Monday to Friday.
Patients need to be seen at least once a year to have prescriptions renewed (or sometimes more often depending on medications and especially after medication dosing changes).
REFERRALS
Please allow 48 to 72 hours for referrals to specialists to be processed.
If you are seen by a specialist without a referral, your insurance company may not cover the cost of the visit and you may be responsible for the entire cost. It is difficult to obtain a referral rectroactively and is not recommended. It is your responsibility to call several days ahead to obtain the referral and NOT on the day of the visit.
EMERGENCY AND AFTER HOURS LINE
Please call MEDLINK at 323-5465 to reach the physician on-call for any urgent matters. If you are having chest pain, difficulty breathing or just feeling very sick, please call 911 or go to the nearest emergency room.
SICK APPOINTMENTS
Please call the office early in the morning for an appointment (phone lines open at 8:30 AM). Most sick patient appointments can be made the same day if you call early enough.
RESCHEDULING APPOINTMENTS/MISSED APPOINTMENTS
If you are unable to make your appointment, please call at least 24 hours in advance to cancel your appointment. This is a courtesy to us and to other patients since your cancellation will free an appointment for a sick patient to be seen.
A fee of $10 will be charged for late cancellations (cancellations < 24 hours).
A fee of $25 will be charged if you do not show up for your appointment.
HOSPITALIZATIONS
Our physicians concentrate on outpatient care. Patients who require admission to the hospital are referred to Hospitalists (physicians who specialize in taking care of patients in the hospital) for inpatient care.
LABORATORY
Our clinic uses both CPL and QUEST labs. If your insurance plan requires a different lab, please inform our office.
LABS AND X-RAY RESULTS
Please allow two weeks for the physician to contact you concerning the results of your routine labs and/or X-ray reports. The physician will either call you or write a letter. If you have not heard from us in two weeks, please call the clinic so we can follow-up on the results.
PHYSICIAN SCHEDULES
Our physicians work part-time. If your primary physician is not available, the other physician will be able to see you or take care of any urgent matters.
OTHER FEES
Fee for FMLA papers, disability papers and any other paperwork that requires the physician's time and signature: $25.
Fee for copy of medical records requested from outside companies: $40.
Fee for copy of medical records requested by the patient: $30 plus 10 cents/page.
There is no fee if the medical records are faxed directly to another physician's office.
Fee for late cancellations (cancellations < 24 hours): $10.
Fee for "no-shows" (if you do not show up for your appointment): $25.
